CERI VOLUNTEER APPLICATION STATEMENT
I understand that this is an application for and not a commitment or promise of volunteer opportunity. I certify that I have and will provide information throughout the selection process, including on this application and in interviews with CERI that is true, correct and complete to the best of my knowledge. I certify that I have and will answer all questions to the best of my ability and that I have not withheld and will not withhold any information contained on my application which I understand and agree will be verified by CERI and I hereby give permission for CERI to contact anyone it deems necessary to investigate or verify any information provided by me to discuss my suitability for a volunteer position, including my background, volunteer experience, education, or related matters. I voluntarily and knowingly waive any and all rights I may have and/or claims against CERI, CERI’s members, directors, trustees, employees, agents, volunteers and other staff, including, without limitation, claims related to defamation, invasion of privacy, or similar cause of action against anyone and which arise in any way as a result fo my completion of this form or the use of any such information that I provide herein. I understand that misrepresentation or omissions may be cause for my immediate rejection as an applicant for a volunteer position with CERI or my termination as a volunteer.
CONFIDENTIALITY: I understand that while volunteering at CERI, information that is shared within CERI is considered confidential. This includes any information related to clients: their health status, their personal lives, or any other information related to them as well as overall information shared within CERI. I agree that under no circumstance will I discuss this information with people outside of CERI.
