CERI BOARD MEMBER APPLICATION STATEMENT
I understand that this is an application for and not a commitment or promise of volunteer board member opportunity. I certify that I have and will provide information throughout the selection process, including on this application and in interviews with CERI that is true, correct and complete to the best of my knowledge. I certify that I have and will answer all questions to the best of my ability and that I have not withheld and will not withhold any information contained on my application will be verified by CERI and I hereby give permission for CERI to contact anyone it deems necessary to investigate or verify any information provided by me to discuss my suitability for a volunteer board member position, including my background, volunteer experience, education, or related matters. I voluntarily and knowingly waive all the rights to bring an action for defamation, invasion of privacy, or similar cause of action against anyone providing such information. I understand that misrepresentation or omissions may be cause for my immediate rejection as an applicant for a volunteer position with CERI or my termination as a volunteer board member.
I agree that, if selected as a volunteer board member, CERI shall at its discretion, and without obligation, except as otherwise required by law, maintain insurance with respect to my actions as a volunteer for CERI in accordance with this statement. I agree that it shall otherwise be my sole responsibility to maintain personal insurance coverage covering me with respect any injuries sustained or otherwise related to my work as a volunteer board member for CERI under this Agreement, including with respect to any actions for which I may have indemnification obligations hereunder.
CONFIDENTIALITY: I understand that while volunteering at CERI and throughout the interview process information that is shared by or within CERI is considered confidential. This includes any information disclosed as to CERI’s processes, products, techniques, strategies, trade secrets,business plans, and forecasts. Also confidential is any information related to CERI clients: their health status, their personal lives, or any other information related to them as well as overall information shared by or within CERI. I agree that under no circumstance will I discuss this information with people outside of CERI during the my time as volunteer board member and thereafter (without expiration).
